Why baseline labs matter before GLP-1 medication
GLP-1 medications are remarkable when they fit; they're also not appropriate for everyone. Labs help your clinician make the right call.
Most members starting semaglutide or tirzepatide are well people who simply want help losing significant weight. For those members, the lab requirements are short — confirm there's no contraindication, establish a baseline, and ship the medication.
But some members have underlying issues that make GLP-1 medications either riskier or less effective than they could be. Subclinical hypothyroidism. Iron deficiency. Vitamin D insufficiency. PCOS. What each marker tells your clinician
HbA1c. Your 3-month glucose average. Catches pre-diabetes and undiagnosed diabetes. Changes the medication choice (Mounjaro/Ozempic for diabetic; Wegovy/Zepbound for non-diabetic chronic weight management).
Fasting glucose and fasting insulin. Together they reveal insulin resistance before HbA1c does. If insulin is high, your clinician may add metformin alongside the GLP-1 for faster progress.
Comprehensive metabolic panel. Kidney function (we need this for dosing) and liver enzymes. Mild fatty-liver-pattern elevations are common in members who have carried weight for years; we monitor them on therapy.
Complete blood count. Anemia is common, especially in menstruating women. Iron-deficient members feel terrible on GLP-1 medications because the appetite suppression makes nutrient intake worse. Worth knowing.
TSH. Subclinical hypothyroidism affects ~5% of adults and dramatically reduces weight loss response. We treat it; weight loss responds.
Lipid panel. Establishes baseline cardiovascular risk. GLP-1 medications typically improve lipids substantially, which is one of their underappreciated benefits.
Vitamin D, B12, ferritin (optional). Three common deficiencies that drain energy and motivation. Cheap to check, easy to fix, big effect on how you feel during the program.

How the lab process integrates with your program
If you're starting a weight-loss program: sign up for the program first. Your clinician will identify which labs you need based on your intake. If you already have recent labs (within 6 months) from your PCP, those are usually fine — you can upload them during sign-up.
If you need new labs, we send a Quest or LabCorp requisition. You walk in for the draw, results come back in 3-5 business days, your clinician reviews them, and your prescription (if appropriate) follows.
On treatment, we re-check key markers at 3 months (HbA1c, lipid panel, liver enzymes) to verify the medication is working and not causing any unexpected effects. This is standard of care and most members find it reassuring.
