PT-141 troches — for libido and arousal that start in the brain

Most ED and libido drugs work on blood flow. PT-141 (bremelanotide) works on the central nervous system — increasing desire and arousal at the source. Compounded as troches by licensed U.S. pharmacies, prescribed by clinicians who know how to dose it.

— Overview

What PT-141 is and why it's different

PT-141 was originally developed as a tanning agent. It became something more interesting when researchers noticed an unexpected side effect.

Bremelanotide (PT-141) is a melanocortin receptor agonist — a peptide that activates melanocortin pathways in the brain. The drug was originally developed by Palatin Technologies as a sunless tanning agent. During clinical trials, researchers noticed an unexpected side effect: increased sexual desire and spontaneous erections. The drug development pivoted, and bremelanotide is now FDA-approved (as Vyleesi) for premenopausal women with hypoactive sexual desire disorder.

The mechanism is what makes PT-141 unusual. Sildenafil, tadalafil, and the entire PDE-5 inhibitor class work peripherally on blood flow — they enable an erection when arousal is already happening. PT-141 works centrally, in the brain, increasing the desire and arousal itself. For members whose issue isn't mechanics but desire — and that's many men and women, particularly with age, SSRI use, or chronic stress — this is a fundamentally different tool.

The FDA-approved Vyleesi requires subcutaneous injection. The compounded troche form (a small lozenge dissolved under the tongue) provides the same active molecule with easier use and no needles. HealifyNow works with licensed U.S. compounding pharmacies that produce pharmaceutical-grade bremelanotide troches.

Who PT-141 fits

💭

Desire, not just mechanics

If the problem is wanting to, not being able to — PT-141 targets the right level.

Works through the same mechanism in both sexes. Used routinely for both.

🤷

PDE-5 inhibitors not enough

For men where mechanical drugs work but desire is still flat, PT-141 fills the gap.

— Mechanism

How PT-141 works

Bremelanotide activates melanocortin 4 receptors (MC4R) in the hypothalamus and other brain regions involved in sexual response. MC4R is a known regulator of sexual behavior in animal models and appears to play a similar role in humans. Activation produces increased sexual desire, increased arousal in response to sexual stimuli, and in men supports the central component of erection (alongside the peripheral blood flow effects of PDE-5 inhibitors).

The fact that PT-141 works in both men and women through the same receptor system is one of its most interesting features. Most sexual function medications target sex-specific anatomy. PT-141 targets the underlying neural drive that's common to both.

The drug has a 30-60 minute onset and 4-8 hour duration. It's used as-needed, before anticipated intimacy, rather than daily. The peptide is broken down in the digestive tract, which is why injection or sublingual (under the tongue) administration is required — the troche bypasses the gut.

One side effect worth knowing: PT-141 can cause a temporary, mild rise in blood pressure for an hour or two after dosing. This is usually clinically insignificant in healthy members but is one reason cardiovascular screening matters.

— Safety

Safety information

Your clinician will review every relevant detail with you. Here are the most important points to know up front.

Blood pressure considerations

PT-141 can transiently raise blood pressure. Members with uncontrolled hypertension or significant cardiovascular disease are typically not candidates.

Nausea is the most common side effect

Roughly 1 in 4 members experience mild nausea, particularly with the first few doses. It typically lessens with continued use.

Skin pigmentation

Because melanocortin receptors also affect pigmentation, some members experience subtle skin darkening, new freckles, or darkening of existing moles with frequent use. Reversible. Monitor any concerning skin changes.

No effect on hormones

PT-141 does not affect testosterone, estrogen, or other sex hormones.

Important

All medications must be prescribed by a licensed provider based on medical necessity. Treatment is not suitable for everyone. Results may vary.

— Use

How PT-141 troches are dosed

Typical dosing is 1-3 mg sublingual troche, taken 30-60 minutes before anticipated intimacy. Your clinician will start you at a conservative dose and adjust based on response and tolerance.

Place the troche under your tongue and let it dissolve fully — don't chew or swallow it. Effects typically begin within 30-60 minutes and last 4-8 hours.

PT-141 is used as-needed, not daily. Most members find a few uses per week is more than enough; many use it once a week or less. Daily or near-daily use is not recommended because of cumulative pigmentation effects.

PT-141 can be combined with PDE-5 inhibitors (tadalafil, sildenafil) for men where both desire and mechanics need addressing. Your clinician will discuss whether the combination fits.

— FAQ

Frequently asked questions

HealifyNow makes it easy to move forward with confidence. Here are the answers to what people ask us most.

Is PT-141 FDA-approved?

+

Bremelanotide is FDA-approved as Vyleesi (injectable form) for premenopausal women with hypoactive sexual desire disorder. Off-label use in men and in postmenopausal women, and compounded troche form, are prescribed by licensed clinicians at their discretion.

How is PT-141 different from Viagra or Cialis?

+

PDE-5 inhibitors work peripherally on blood flow — they enable an erection when arousal is happening. PT-141 works centrally in the brain — it increases the desire and arousal itself. Different mechanism, different problem solved. Many men benefit from using both.

Can women use PT-141?

+

Yes — bremelanotide is FDA-approved for women with low desire (as Vyleesi), and the same molecule in troche form works similarly. Many women find it effective.

How often can I use PT-141?

+

As-needed, generally not more than a few times per week. Daily or near-daily use can accumulate pigmentation effects and is not recommended.

Will PT-141 work with antidepressants?

+

Yes — many members use PT-141 specifically because SSRIs or SNRIs have damped their sexual function. The mechanism is independent of the serotonin system that drives antidepressant sexual side effects.

Does PT-141 cause hair growth or change skin color permanently?

+

Some members notice subtle skin darkening, new freckles, or darkening of moles with frequent use. These effects are reversible and gradually fade after stopping the drug. We monitor for concerning skin changes.

Can I take it if I have high blood pressure?

+

If your blood pressure is well-controlled on medication, PT-141 is generally safe. If your blood pressure is uncontrolled or you have significant cardiovascular disease, your clinician may not prescribe it.

How quickly does it work?

+

Onset is typically 30-60 minutes after sublingual dosing. Effects last 4-8 hours.
