In 2024, the FDA expanded Wegovy's indication based on the SELECT trial results, which showed that semaglutide reduced major adverse cardiovascular events by 20% in adults with obesity and pre-existing cardiovascular disease. This was a landmark expansion — semaglutide is now positioned as cardiovascular preventive therapy in addition to weight management.
Wegovy and compounded semaglutide contain the same active molecule. Wegovy is the brand-name product manufactured by Novo Nordisk; compounded semaglutide is prepared by licensed U.S. compounding pharmacies. The 2.4 mg target dose is higher than Ozempic's diabetes dosing (up to 2 mg) — which is the dose at which clinical trials demonstrated the consistent ~15% weight loss. Lower doses produce lower weight loss; higher doses haven't been shown to add benefit and increase side effects.
The titration schedule (16-week ramp-up to 2.4 mg) exists to minimize GI side effects. Members who climb too fast often experience nausea that pushes them off therapy. Slower titration in some members may improve tolerance.
