1. Our commitment to your privacy

HealifyNow and the independent licensed clinicians who provide care through the platform are committed to protecting your protected health information ("PHI"). PHI is information that can identify you and relates to your past, present, or future health, treatment, or payment for care. We are required by law to maintain the privacy of your PHI, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

2. How we may use and disclose your health information

We may use and disclose your PHI for the following purposes without your written authorization:

  • Treatment. To provide, coordinate, and manage your care — for example, sharing your information with a clinician, pharmacy, or laboratory involved in your treatment.
  • Payment. To bill and collect payment for the services you receive, including verifying coverage and processing transactions through our payment partners.
  • Health care operations. To support quality review, clinician oversight, training, and the general administration of the platform.
  • As required by law. When federal, state, or local law requires the use or disclosure, including public health activities and reporting suspected abuse or neglect.
  • To avert a serious threat. To prevent or lessen a serious and imminent threat to the health or safety of you or the public.

3. Disclosures that require your authorization

Most uses and disclosures of psychotherapy notes, uses and disclosures for marketing purposes, and any sale of PHI require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.

4. Your rights regarding your health information

You have the following rights with respect to your PHI:

  • Right to access. You may inspect and request a copy of the health information used to make decisions about your care.
  • Right to amend. You may request a correction to health information you believe is inaccurate or incomplete.
  • Right to an accounting of disclosures. You may request a list of certain disclosures we have made of your PHI.
  • Right to request restrictions. You may ask us to limit how we use or disclose your PHI, though we are not required to agree in all cases.
  • Right to confidential communications. You may ask us to contact you in a specific way or at a specific location.
  • Right to a paper copy. You may request a paper copy of this notice at any time.

5. Our responsibilities

We are required to maintain the privacy and security of your PHI, to notify you promptly if a breach occurs that may have compromised the privacy or security of your information, and to follow the duties and privacy practices described in this notice. We will not use or share your information other than as described here unless you tell us we may in writing.

6. Changes to this notice

We reserve the right to change this notice and to make the revised notice effective for health information we already have as well as any information we receive in the future. The current notice will always be posted on this page with its effective date.

7. Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at team@healifynow.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
