How beta blockers work mechanistically

Beta-1 receptors are concentrated in the heart. Activation by adrenaline or norepinephrine increases heart rate and contractile force. Beta-1 blockade slows the heart, reduces contractile force, and reduces oxygen demand on the heart muscle — therapeutic in coronary disease, heart failure, and many arrhythmias.

Beta-2 receptors are concentrated in bronchi, blood vessels, and some smooth muscle. Activation dilates bronchi and dilates certain blood vessels. Beta-2 blockade can constrict bronchi (one reason non-selective beta blockers can be problematic in asthma) and reduce peripheral tremor (because some tremor is mediated by beta-2 in muscle spindles).

For performance anxiety, the mechanism is straightforward. The physical symptoms of anxiety — racing heart, trembling hands, dry mouth, voice quaver — are mediated by adrenaline. Blocking the physical symptoms doesn't eliminate the psychological experience of nervousness, but it removes the feedback loop where the body's physical response makes the anxiety worse. For migraine prevention, the mechanism is less clear. Several theories exist — central effects on serotonin systems, vascular effects, or modulation of cortical excitability.
