What sets the longevity panel apart
Same lab partners as the standard full panel, but a markedly different selection of biomarkers and a longer clinician review.
The longevity panel includes everything in the full panel, plus:
Advanced cardiovascular markers. ApoB (atherogenic particle count), Lp(a) (genetic CV risk), LDL-P (particle number), hsCRP, fibrinogen, lipoprotein-associated phospholipase A2 (Lp-PLA2), and homocysteine. Together these give a picture of cardiovascular trajectory that's substantially more useful than the standard LDL number.
Insulin and metabolic detail. Fasting insulin, HOMA-IR, HbA1c, fructosamine (a 2-3 week glucose average), and adiponectin (a fat-cell hormone strongly linked to longevity). Catches dysfunction years before glucose alone shows it.
Comprehensive hormone panels. Total and free testosterone, estradiol, progesterone (women), DHEA-S, SHBG, cortisol (AM and PM if useful), full thyroid including reverse T3, IGF-1, prolactin. Hormones drive symptoms and they drive long-term healthspan.
Inflammation and immune markers. hsCRP, IL-6 where indicated, ferritin (which is also an inflammation marker), white blood cell differential, neutrophil-lymphocyte ratio.
Biological age testing (optional add-on). Epigenetic methylation panels — TruDiagnostic TruAge, DunedinPACE — that estimate biological age and pace of aging from DNA methylation patterns in blood. Useful as a tracked biomarker that responds to interventions.
GlycanAge or aging-rate alternatives are available as add-ons for members specifically interested in tracking the speed of biological aging.

How biological age testing works
Epigenetic age tests measure DNA methylation — chemical modifications to DNA that don't change the sequence but do change gene expression. Methylation patterns at specific sites across the genome change predictably with age, and the rate of change differs between individuals based on lifestyle, environment, and biology.
The first-generation tests (Horvath, Hannum) estimate chronological age from methylation. The current-generation tests (PhenoAge, GrimAge, DunedinPACE) are trained against health outcomes — they estimate biological age that predicts mortality and morbidity, not just chronological age. DunedinPACE specifically estimates the pace of aging — how fast you're aging biologically right now, which is the most actionable metric.
These tests have improved substantially in the last 3-5 years and are now stable enough that intervention-driven changes are measurable over 6-12 months. They're not magic — a single test result has noise and shouldn't be over-interpreted — but the trend over multiple tests on the same person is genuinely informative.
We offer biological age testing as an optional add-on rather than a centerpiece. The standard longevity panel measures the inputs you can act on; biological age testing measures the integrated output that those interventions move.

Fasting required
10-12 hour fast for accurate metabolic markers. Water only.
Optional separate blood draw for bio-age
TruDiagnostic and similar tests use their own kits — separate draw at the same Quest or LabCorp visit.
Bio-age noise
Single bio-age results have meaningful test-retest variability. The trend across 2-3 tests is what's informative.
Cash-pay service
Insurance does not cover longevity lab work. Cost is transparent up front.
All medications must be prescribed by a licensed provider based on medical necessity. Treatment is not suitable for everyone. Results may vary.
How the longevity panel works
Order the panel online and choose any optional add-ons (biological age testing, GlycanAge, additional hormones). Within a few business days, you receive a lab requisition you can present at any Quest or LabCorp center.
Fast for 10-12 hours, then walk in for the draw. The draw takes 5-15 minutes. Standard markers post in 3-5 business days; epigenetic tests typically take 3-4 weeks.
Once all results are in, you book your clinician review — 45 minutes by video. The clinician walks through every marker, explains the patterns, and builds your written protocol based on what the results show. If you're a longevity program member, the review and follow-up are included in your monthly fee.
Re-testing is recommended at 90 days for any markers actively being intervened on, and annually for the full panel.

How is this different from the standard full panel?

The full panel covers comprehensive health screening. Is biological age testing actually useful?

The current-generation tests (DunedinPACE, GrimAge) have improved substantially and are now stable enough to track intervention effects over 6-12 months. We recommend them as a tracked biomarker, not as a one-time vanity metric. Most useful when run 2-3 times over a year or two on the same person.
How often should I retest?

Full longevity panel annually. Specific markers being actively intervened on (HbA1c, ApoB, hsCRP, hormone levels on replacement) every 3-6 months. Biological age every 12 months at most.
Will insurance cover any of this?

Generally no. The basic markers (CBC, lipid panel) may be covered if your PCP orders them through your insurance, but most longevity-specific markers and the biological age tests are cash-pay.
Is this enough to skip an annual physical?

No. A physical exam, blood pressure measurement, skin check, and basic preventive care still matter. The longevity panel complements your PCP relationship rather than replacing it.
What if results show a serious issue?

Your clinician will explain what's significant, what needs immediate action, and what may need specialty referral. We coordinate with cardiologists, endocrinologists, and other specialists when results call for it.
Can I do this if I'm already on hormone replacement?

Yes. Hormone levels on HRT or TRT are an important part of the panel — the goal is to verify the protocol is working at the right levels and adjust if not.
