Mounjaro (tirzepatide) received FDA approval in 2022 for type 2 diabetes. It's the first dual GIP/GLP-1 receptor agonist in the US market. The SURPASS trial program demonstrated superior HbA1c reduction (~2.0-2.5%) and superior weight loss (~15-20%) compared to semaglutide head-to-head.

Tirzepatide activates two incretin pathways. GLP-1 receptor activation is the same mechanism semaglutide uses. GIP (glucose-dependent insulinotropic polypeptide) receptor activation adds a different and complementary effect — enhanced insulin sensitivity, increased lipolysis, and additional weight loss effects.

How Mounjaro works

GLP-1 receptor activation. Same mechanism as semaglutide — slowed gastric emptying, appetite suppression, glucose-dependent insulin release, glucagon suppression.

GIP receptor activation. Distinct from GLP-1. GIP activation enhances insulin sensitivity, increases lipolysis (fat breakdown), and appears to add weight-loss effect beyond GLP-1 alone.

Net clinical effect. In SURPASS-2 head-to-head trial, tirzepatide 5/10/15 mg produced greater HbA1c reductions and greater weight loss than semaglutide 1 mg at 40 weeks. The 15 mg dose averaged ~12% body weight loss in diabetic patients.

Cardiovascular outcome data is still maturing. The SURPASS-CVOT trial is ongoing; SURMOUNT trials have shown favorable cardiovascular markers.

How to use Mounjaro

Mounjaro is dispensed as single-dose pre-filled pens or vials. Once weekly subcutaneous injection.

Titration: 2.5 mg for 4 weeks, then 5 mg. Increase by 2.5 mg every 4 weeks based on glucose control and tolerance, to maximum 15 mg weekly. Most members do well at 7.5-12.5 mg for diabetes management.

Refrigerate; can be at room temperature 21 days once in use. Inject into abdomen, thigh, or upper arm; rotate sites.
