How tadalafil works

Erections begin with sexual arousal triggering nitric oxide release in the corpus cavernosum (the erectile tissue of the penis). Nitric oxide activates an enzyme (guanylate cyclase) that produces cGMP, which causes smooth muscle to relax. Relaxed smooth muscle in the corpus cavernosum lets blood flow in, producing an erection.

cGMP is broken down by phosphodiesterase type 5 (PDE-5). Tadalafil inhibits PDE-5, so cGMP sticks around longer, smooth muscle stays relaxed longer, and the erectile response is amplified and prolonged. Crucially, tadalafil doesn't cause erections by itself — it requires sexual arousal to trigger the cascade. This is why members on PDE-5 inhibitors don't walk around with persistent erections.

PDE-5 also exists in smooth muscle elsewhere in the body, particularly the bladder neck and prostate. Daily tadalafil relaxes this smooth muscle too, which is the mechanism for the BPH benefit.

The 17-hour half-life — much longer than sildenafil's 4 hours — is what produces the 36-hour effective window. Daily dosing at 2.5 mg or 5 mg builds steady tissue levels without the spike-and-trough pattern.
