How HRT works
Estradiol replacement. Transdermal forms (patches, gels) bypass the liver, avoiding the small VTE risk associated with oral estrogens. Doses are titrated to symptom relief and to physiologic levels — typically estradiol levels in the 40-100 pg/mL range. Symptom relief usually emerges within 2-6 weeks.
Progesterone. Required for women with a uterus to protect the endometrium from estrogen-driven proliferation. Bioidentical micronized progesterone (Prometrium) is preferred — taken at bedtime, where it also produces calming and sleep effects.
Testosterone. Off-label for women in the US but used routinely by HRT specialists. Low-dose testosterone cream or troche supports libido, energy, motivation, muscle, and bone. Dosed well below male replacement levels.
Vaginal estrogen. For women with primarily genitourinary symptoms (dryness, discomfort, recurrent UTIs), vaginal estrogen is highly effective with minimal systemic absorption.
Non-hormonal options. SSRIs/SNRIs for hot flashes, gabapentin for sleep, oxybutynin for night sweats, CBT adapted for menopause. For women who can't or don't want HRT.

Isn't HRT dangerous because of the WHI?

The WHI was misinterpreted. The increased risks in that study were primarily in older women starting HRT well after menopause, on oral conjugated estrogens with synthetic progestin. For healthy women within 10 years of menopause, modern transdermal HRT with bioidentical progesterone has a favorable risk-benefit profile.
How long can I stay on HRT?

Will HRT cause weight gain?
