What longevity medicine actually is

Strip away the marketing and you are left with a small set of interventions that have real evidence behind them in humans, and a longer list with strong evidence in animals.

Longevity medicine, sometimes called geroscience or healthspan medicine, is the clinical application of what we know about aging biology. The premise is simple: aging is a set of biological processes — cellular senescence, mitochondrial decline, chronic inflammation, loss of proteostasis, telomere attrition, and several others — that we are beginning to understand well enough to slow, in some cases. The goal is not to live to 150. The goal is to spend more of your life in good health, with the body and brain you actually want to inhabit.

The field has matured fast. Ten years ago, longevity protocols were dominated by supplements with thin evidence and a lot of hand-waving. Today, there are FDA-approved medications being studied for off-label longevity use — rapamycin, metformin, acarbose, SGLT2 inhibitors — and a far better grasp of which lab markers actually matter. Continuous glucose monitoring, advanced lipid panels, inflammatory markers like high-sensitivity CRP and homocysteine, hormone panels including DHEA-S and SHBG, and biological age testing through epigenetic methylation panels have all moved from research labs into the clinic.

What we actually intervene on

Metabolic health. Insulin sensitivity is arguably the single most important lever in longevity. Rising fasting glucose, rising fasting insulin, and post-meal glucose excursions all predict downstream disease decades before a diabetes diagnosis. Metformin (the most-prescribed longevity drug worldwide), acarbose, GLP-1 agonists at low doses, SGLT2 inhibitors in select patients, and continuous glucose monitoring all sit in this category.

The mTOR pathway. Rapamycin remains the only pharmacological intervention to consistently extend median and maximum lifespan in mammals in independent labs. Low-dose intermittent rapamycin is the most well-developed longevity protocol with real human follow-up data emerging now. It is not appropriate for everyone, and the discussion is one a clinician needs to have with you.

Mitochondrial function and NAD+ biology. NAD+, the universal energy currency in cells, declines with age. NMN, NR, methylene blue, NAD+ injections and nasal sprays are all attempts to support mitochondrial function. The human evidence for big lifespan effects is still limited; the evidence for symptomatic energy improvements in many people is reasonable.

Hormonal optimization. Testosterone, estrogen, progesterone, thyroid hormones, and DHEA all decline in patterns specific to each person. Treating clinical deficiency where it exists has a large effect on healthspan — energy, cognition, mood, sleep, bone, muscle.

Cardiovascular risk. ApoB, Lp(a), advanced lipid panels, and aggressive management of even modestly-elevated LDL extends life by years. The data here is the strongest in all of medicine; many longevity protocols underweight this.

Inflammation and immune balance. Chronic low-grade inflammation drives everything from atherosclerosis to neurodegeneration. Measuring it (hsCRP, fibrinogen, IL-6) and addressing root causes — dental health, gut microbiome, visceral fat, sleep apnea — is unglamorous but high-leverage.

Sleep, stress, and movement. No medication will offset a chronically broken sleep architecture. Longevity care without a serious look at sleep, recovery, training load, and stress regulation is incomplete.

How HealifyNow's longevity program works

Step 1 — Free intake call. A care team member spends 15-20 minutes understanding what you want from the program, what you have already tried, and what your current health picture looks like. This is also where we tell you honestly if we are not the right fit.

Step 2 — Comprehensive baseline labs. Most members start with a comprehensive panel: full lipid panel including ApoB and Lp(a), advanced metabolic markers including fasting insulin and HbA1c, full hormone panel, inflammatory markers, full thyroid, full iron studies, vitamin D, B12, homocysteine, GGT, and where appropriate biological age testing. We order labs through Quest or LabCorp; you go to a draw center near you.

Step 3 — Clinician review and protocol. Your clinician reviews everything, has a 30-45 minute video visit with you, and builds a written protocol. This will typically include: any prescription medications appropriate for you, any supplements with strong-enough evidence to be worth taking, and the lifestyle interventions that are highest leverage for your specific labs.

Step 4 — Implement. Prescriptions ship from licensed U.S. pharmacies. Your clinician is reachable through the platform during implementation if anything comes up.

Step 5 — Re-test and adjust. Most members re-test labs at 90 days and every six months thereafter. The protocol evolves with the data, not against it.

Do I need to be unhealthy to benefit?

No — in fact, healthy people in their 30s and 40s tend to get the most leverage from longevity care because they are still upstream of disease. Catching insulin resistance, elevated ApoB, or low testosterone before they cause symptoms is much easier than reversing those problems later.

Is rapamycin part of every longevity protocol?

No. Rapamycin is appropriate for a subset of members based on age, lab profile, and clinical picture. Many members do well without it. Your clinician will only recommend it if the risk-benefit case is clear for you.

Do you sell supplements?

We don't run a supplement store. When your clinician recommends a specific supplement, they tell you exactly which form and which brand has the data, and you buy it wherever you like. We make no money from supplement sales.

How is this different from a concierge medical practice?

Concierge practices typically cost $5,000-$15,000 per year and are in-person. Our longevity program is online-first, costs a fraction of that, and is focused specifically on healthspan rather than general primary care. Many members keep their regular PCP and add HealifyNow for the longevity side.

Will my insurance cover any of this?

Some lab work may be covered if you have your PCP order it through traditional insurance. The clinician visits, the program fee, and most longevity-specific medications are paid out of pocket. We are transparent about cost up front.

How long until I see results?

What if I'm taking medications already?

Is biological age testing worth doing?

Epigenetic age tests like Horvath, DunedinPACE, and TruDiagnostic provide an interesting biomarker that responds to interventions, but the field is still maturing. We offer it as an optional add-on; we don't make it the centerpiece because the actionability of the result is still limited.
