How topical finasteride works
Local 5-alpha reductase inhibition. The scalp expresses 5-alpha reductase locally, converting testosterone to DHT in the follicular environment. Topical finasteride inhibits this enzyme where it's applied, reducing local DHT without significantly affecting systemic levels.
Systemic absorption. Studies of topical finasteride show approximately 5-10% systemic absorption compared to an equivalent oral dose. Serum DHT reduction is typically 30-50% with topical, versus 70%+ with oral. The clinical outcome on hair, however, is comparable because the relevant DHT exposure is local to the scalp.
Combination effect. Topical finasteride is often compounded with topical minoxidil. Finasteride blocks DHT (the cause of miniaturization), minoxidil extends the growth phase and increases follicle size. Together, they address the hair-loss process from two different angles. Some compounds add latanoprost (prostaglandin analog that supports follicle strength) or ketoconazole (anti-inflammatory).
